Alternate font macros implemented.

Hello!

I have implemented macros .BR, .IR, .RB, .RI, .BI and .IB in man2hlp. 
They are currently used in the Polish manual.

The idea of those macros is that they use alternate fonts for every word
(quoted text counts as one word).  The spaces outside quotes are
removed.

So, if you want to write:

Confirm by entering the word yes.

making "yes" italic, you can now write in the manual:

Confirm by entering the word
.IR yes .

instead of the current ugly variant:

Confirm by entering the word
.I yes
\&.

which would leave a space before the dot.
